The Health Educator (HE) educates individuals and communities about health topics, promoting healthy habits, and disease prevention. The Health Educator works with patients in the RWB YouPLUS and PAWH 50PLUS programs and provides key education to clients living with HIV/AIDS about HIV transmission and how to reduce the risk of HIV transmission.

As part of the essential functions of this role, the Health Educator:

• Maintains a calendar of monthly group offerings and leads health education classes and groups for enrolled patients of RWB YouPLUS and PAWH 50PLUS
• Engages patients through individual connection and education and encourages them to attend groups that could benefit them
• Assists with nutrition, exercise, and other wellness practices, especially for older individuals with chronic illnesses
• Provides sexual health education to enrolled patients
• Teaches self-advocacy and ensures clients can manage multimorbidity, polypharmacy, preventative care and more
• Communicates with medical providers about adherence, behavioral health, and other needs
• Conducts group and individual sessions that focus on strategies for achieving positive health outcomes
• Uses a comprehensive holistic approach to care
• Conducts Intake/Assessments that must identify the need for Health Education services and confirmation of ongoing need through reassessment completed every six months
• Maintains records for all clients enrolled in this service and makes them available for review by NYSDOH AI staff
• Completes and documents date, time, and duration of encounters, including face to face, telephone, virtual, or other platforms
• Addresses topics, goals, and objectives with clients in relation to Health Education
• Establishes evidence of pre- and post-tests to evaluate knowledge gained in group sessions, and an indication of knowledge gained from individual sessions
• Documents how the session promoted and supported client access to care, adherence to treatment and improved medical outcomes

Qualified Candidate will have Bachelor’s degree in health, human, or education services with one (1) year of qualifying professional experience OR Associate’s degree in health, human, or education services with two (2) years of qualifying professional experience ( qualifying professional experience means verifiable full- or part-time experience in care coordination with the following populations: persons with a chronic illness, and/or persons with a history of mental illness, homelessness, or chemical dependence. Experience with families preferred.) Candidate must be able to communicate clearly and professionally in writing and verbally, demonstrate familiarity with computers and MS Office software (Outlook, Word) and have excellent customer service, interpersonal, time management and organization skills. Candidate will demonstrate an awareness of and sensitivity to the issues that underserved populations, people living with HIV and people who use drugs face, including health disparities that affect people of color at a disproportionate rate.
