Home Care Registered Nurse (PACE) – Bronx, NY (Onsite)
Salary: $112,000–$118,000 annually (commensurate with experience)

Our client is a well-established Medicare and Medicaid PACE program dedicated to helping seniors receive nursing home–level care while continuing to live safely and comfortably in their own homes. Key Responsibilities
Under the direction of nursing leadership, the Registered Nurse is responsible for planning, coordinating, implementing, and evaluating nursing care during assigned shifts, including:

• Receiving and reviewing shift reports to ensure continuity of care
• Monitoring residents’ conditions, behaviors, symptoms, and progress, and communicating findings to nursing leadership
• Administering and documenting medications in accordance with physician orders, policies, and Department of Health regulations
• Performing treatments per physician orders and established protocols
• Collaborating with the Nurse Manager/Charge Nurse on medication or treatment changes
• Ordering medications, coordinating with pharmacy services, and ensuring accuracy of medications and dosages

Qualifications

• Active New York State Registered Nurse (RN) license
• Associate Degree in Nursing required
• BSN preferred
