Job Description

1. Board Certification & Licensing:

   • Must be Board Certified/Board Eligible in Radiation Oncology (MD or DO).
   • Active Illinois (IL) medical license and IL DEA/CSR are required.

2. Schedule & Call Expectations:

   • Monday Friday, 8:00 AM 4:30 PM .
   • Some potential calls depending on the confirmed schedule and credentialing.

3. Travel & Reimbursement:

   • Driving Distance: Round-trip mileage & lodging covered.
   • Flights: Indicate necessity on the Stratum form if applicable

4. Reference Requirements:

   • At least two professional references must be completed at the time of presentation.
   • References should be from recent colleagues or medical directors who can attest to the provider s skills, experience, and professionalism.
