The Role:
This role is 70% in person, 30% remote.
We are seeking a compassionate and dependable Registered Nurse to join our growing home health care team as an RN Case Manager in Fort Collins, CO. In this role, you will collaborate closely with families to develop personalized care plans and deliver skilled nursing services.
Responsibilities:
Conduct home visits every 60 days to assess patient needs and progress.
Develop and implement comprehensive care plans tailored to the unique needs of children with IDD.
Educate and support parents on their child's health, providing coaching as needed.
Utilize telehealth solutions to enhance communication with the care team and families.
Advocate for superior health outcomes for children and advance our mission.
Requirements:
Current RN license in Colorado or compact state, with at least 2 years of nursing experience.
Experience in home care settings is essential; pediatric experience preferred.
CPR certification required; BLS and PALS certifications preferred.
Ability to thrive in a dynamic, startup environment.
Must reside in or be willing to relocate to Fort Collins, CO.

Daily Schedule:
Monday: Mandatory nurse meeting in Denver office (9am-10:30am); rest of day for administrative tasks from home.
Tuesday-Friday: Conduct in-person recertifications (up to 3 per day) and start-of-care assessments (up to 3 per week); remainder of time dedicated to remote charting, family communication, and case management.
