Position Overview

We are seeking a passionate and experienced Health Coach to join our dedicated care team. As a Health Coach, you will work collaboratively with our primary care physicians, care coordinators, and nutritionists to support our clients' wellness journeys and help implement personalized care plans.

Key Responsibilities

• Conduct one-on-one coaching sessions (30 minutes) with clients to support their health and wellness goals
• Work closely with the dedicated care team to develop, implement, and adjust personalized wellness plans
• Provide education and support on lifestyle modifications, including nutrition, exercise, stress management, and sleep hygiene
• Monitor clients' progress and communicate effectively with the care team to ensure integrated care
• Motivate and inspire clients to adhere to their personalized care plans and achieve their health goals
• Stay current with the latest research and trends in health coaching and wellness

Qualifications

• Bachelor's degree in Health Sciences, Exercise Science, Nutrition, or related field (Master's degree preferred)
• Certified Health Coach from a recognized institution (e.g., National Board for Health and Wellness Coaching, American Council on Exercise)
• Minimum of 3-5 years of experience in health coaching, preferably in a medical or high-end wellness setting
• Strong teamwork skills and experience collaborating in multidisciplinary healthcare settings
• Excellent communication, interpersonal, and motivational skills
• Demonstrated passion for helping people improve their health and well-being
• Proficiency in using telehealth platforms and digital health technologies
• Experience working with high-net-worth individuals is a plus
