Here at Homeside Rehab we provide quality Physical, Occupational, and Speech/Swallowing Therapies for patients in their homes. We also assist in coordination of care to ensure that homebound patients remain safe and that all of their medical needs are managed efficiently.

*QUALIFICATIONS/REQUIREMENTS:*
* NYS License (Required)
* Basic computer literacy skills
* 1 year Home Health, Outpatient, SNF experience preferred.
* Evaluate and develop a plan of care according to the patients' prior and current functionality.
* *Good communication* between the office staff and therapists.
* Treat patients with therapeutic practices designed to relieve pain, increase mobility, restore normal function, and maintain maximum performance
* Maintain accurate and *timely* documentation of individualized care
* Collaborate with clinical staff to develop treatment plans that meet the needs of patients
* Adhere to industry standards and medical guidelines
