Job Description Summary: Verifies inpatient prescriber orders, dispenses ordered medications, compounds IV and all other sterile products, prepares and delivers all patient orders, provides drug information and clinical patient review for safety and accuracy, and supervises technicians and interns. Provides support on patient codes and emergency room level I traumas. Demonstrate competencies defined in Inpatient Staff Pharmacist duties and responsibilities in providing age-appropriate care to patients from the age of birth through 21 years and select adult patients.
Job Description: Essential Functions: - Dispenses inpatient medications pursuant to prescriber orders, including determining the appropriateness of medication orders using information such as patient age, weight, disease state(s) and pertinent past medical history, reviews all possible adverse drug events, and considers patient allergies. Determines appropriateness of medication orders and contacts prescribing practitioner if in doubt of appropriateness of order. Verifies medications orders to assure timely and accurate delivery in a form appropriate for the patient.
- Provides information to health care practitioners, caregivers, or patients on the appropriate administration of medications, medication dosing, and medication selection.
- Provides all necessary pharmaceutical care in support of codes and Level I traumas.
- Documents direct patient care, clinical, operational, and financial outcomes and activities appropriately in the medical record.
- Determines appropriateness of medication orders and contacts prescribing practitioner if in doubt of appropriateness of order. Verifies medications orders to assure timely and accurate delivery in a form appropriate for the patient. Further, helps operationalize recommendations through order verification as appropriate and as determined by team leadership.
- Focuses on accurate maintenance of the 'prior to admission' medication list and ensures accurate and timely communication to multidisciplinary team, ambulatory pharmacy, and patient/family regarding discharge medication-related needs.
- Provides in-service training to medical, nursing, and pharmacy staff. Precepts and/or supervises pharmacy residents, students, interns, technicians, or graduates awaiting licensure. Provides appropriate oversight and input in performance evaluations.
Education Requirement: - Bachelor's Degree from an accredited college of pharmacy, required.
- Doctor of Pharmacy, preferred.
Licensure Requirement: Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio, required.
Experience: Pediatric hospital experience or internship, preferred.
